The player explores a series of procedurally generated caves. Starting with a whip, some supply of ropes and a few bombs, the player must go through all caves with one life, defeating monsters, obtaining better equipment, avoiding traps and rescuing women in distress.

The game has a serious level of difficulty, forcing you to start all over again after each death. The protagonist is given very little health, the chances of replenishing which are limited. There is no save function in Spelunky, but shortcuts to future levels can be purchased.

Various goods can be purchased in stores on the map. The shops are different, including those specializing in bombs, weapons, gambling, and even the provision of the services of prostitutes. There is the possibility of acquiring a weapon and eliminating the owner with it - then you can get free items and a certain amount of ownerless money. Murder of a merchant is fraught with getting the protagonist on the "wanted list" and correspondingly negative attitude of other merchants, expressed, no less, in a shot from a shotgun.

- Gun shops offer a random assortment of firearms or bows.
- Bomb stores sell bombs or upgrades for them.
- A wide variety of items are available in general stores, from ropes and bombs to various upgrades (cape to slow down falls, boots for high jump, glove for improved throws, glove for climbing, etc.).
